Summary of strengths:

I can recognize well signs and symptoms of psychopathology, perform mental status examination, perform and interpret functional assessment, document appropriately and correctly, give health promotion and prevention education, maintain professional boundaries and therapeutic relationships, collaborate with the multidisciplinary team members effectively in the spirit of interprofessional collaboration, identify and solve ethical and legal dilemmas, show empathy and non-judgmental practice, and practice within my scope of practice as per my job description. I am also able to do and interpret a psychosocial assessment, prescribe the correct medications for the patient and assess their progress, identify therapeutic approaches and perform psychotherapy effectively according to age while forming a plan of treatment, and develop an individualized therapy plan. Finally, my other strengths also include developing SMART practicum goals and evaluating outcomes.

 Opportunities for growth:

A number of opportunities for growth still exist for me given that there are some areas of clinical skills that I still feel a beginner at and require supervision to perform. These areas presenting opportunities for growth for me include identifying the difference between pathophysiological and psychopathological conditions, doing and interpreting the results of a complete physical examination, prioritizing differential diagnoses, performing DSM-5 diagnoses, and differentiating between normal and abnormal signs and symptoms of psychopathology. Lastly but not least, I also still have an opportunity for improvement in choosing, performing, interpreting, and documenting the results of assessment and screening tools as appropriate for each age group across the lifespan. This is also in the context of knowing when to refer to another specialty based on the screening results.

Now, write three to four (3–4) possible goals and objectives for this practicum experience. Ensure that they follow the SMART Strategy, as described in the Learning Resources.

1.      Goal: Differentiating between pathophysiological and psychopathological conditions.

a.      Objective: By the end of the first practicum month, I will have been able to differentiate between pathophysiological and psychopathological conditions in 15 cases under the supervision of my preceptor.

b.      Objective: By the end of the second month, I will be able to differentiate between pathophysiological and psychopathological conditions in at least 20 cases with minimal supervision.

c.       Objective: By the end of the practicum, I will have been able to differentiate between pathophysiological and psychopathological conditions in at least 25 cases without supervision.

2.      Goal: Coming up with appropriate DSM-5 diagnoses and then prioritizing differential diagnoses.

a.      Objective: By the end of the first practicum month, I will be able to perform the DSM-5 diagnoses of at least 20 patients with supervision.

b.      Objective: By the end of the second month of practicum, I will be able to arrive at a DSM-5 diagnosis and prioritize differential diagnoses for 25 patients with minimal supervision.

c.       Objective: By the end of the practicum semester, I will have been able to make a DSM-5 diagnosis and prioritize differential diagnoses for 25 patients without supervision.

3.      Goal: Performing and interpreting the results of a complete physical examination.

a.      Objective: By the end of the first month of practicum, I will be able to perform and interpret the results of a complete physical examination for 20 patients with supervision from my preceptor.

b.      Objective: By the end of the second month of practicum, I will be able to perform and interpret the results of a complete physical examination for 25 patients with minimal supervision from my preceptor.  clinical skills self-assessment Essay

4.      Goal: Choosing, performing, interpreting, and documenting the results of assessment and screening tools as appropriate for each age group across the lifespan.

a.      Objective: By the end of the first month of practicum, I will be able to choose, perform, interpret, and document the results of assessment and screening tools for five children, five adolescents, five adults, and five elderly patients with supervision.

b.      Objective: By the end of the second month of practicum, I will be able to choose, perform, interpret, and document the results of assessment and screening tools for five children, five adolescents, five adults, and five elderly patients with minimal supervision.

c.       Objective: By the end of the practicum, I will have been able to choose, perform, interpret, and document the results of assessment and screening tools for ten children, ten adolescents, ten adults, and five elderly patients without supervision.

Target Patient Population:   Across the Lifespan with individuals, groups, and families
