Evidence-Based Practice Proposal Section D: Use of the Roger’s Diffusion of Innovations Theory to Effect Falls Prevention Change

The previous paper in this evidence-based practice (EBP) project described the solution to the earlier stated problem of patient falls in detail. In particular, it identified through a search of evidence from scholarly literature a bundled intervention comprising of the three elements of purposeful hourly rounding, use of bed alarms, and nurse training (McCarthy, 2016; Leone & Adams, 2015). This set the stage for the actual implementation of the interventions within the healthcare setting in question. But to do that there must be a change model chosen to guide and facilitate the process. In this instance, the change model preferred is the Roger’s Diffusion of Innovations theory or model of change (Dearing & Cox, 2018). The purpose of this paper is therefore to outline how the Roger’s DOI theory of change can be applied to implement the identified EBP interventions aimed at preventing and reducing the incidence of accidental patient falls among elderly patients.


The Selected Change model and Its Relevance to the Project

As stated in the introduction above, the selected change model for this project is the Roger’s Diffusion of Innovations model. This is a change model that comprises of five important steps that progressively facilitate the smooth introduction of the impending change in the organization. These steps are (i) knowledge, (ii) persuasion, (iii) decision, (iv) implementation, and confirmation. Each of these steps is discussed in detail below with regard to this particular project. Roger’s DOI model suits this project because it particularly divides the employees into various calibers. This division of the employees is very helpful in the targeted management of barriers such as resistance to change (offered by the late majority and laggards).

According to Dearing and Cox (2018), innovators are those employees that are proactive in clinical inquiry and propose change. They are only 2.5% of the employees. Then there are the early adopters representing 13.5% of the employee population. These are the employees that quickly accept and implement the proposed change because they understand research and the process of clinical inquiry. These are usually baccalaureate nurses educated and trained in research. The next group are the early majority who are indeed the majority of the employees at 34%. This group of employees will adopt the change but need some convincing by a trusted colleague or a person of influence within the organization. After this group follow the difficult groups. These are represented by the late majority who are also 34% of the employee population. They are resistors to change and will require a lot of pushing and convincing from the management as well as the innovators and early adopters to embrace the change. Finally is the most difficult group known as the laggards. They are just 16% of the employee population but are fiercely resistant to change. Applying the Stages of the Roger’s DOI Change Model to the Proposed Interventions

It has been mentioned above that Roger’s DOI change model has got five distinct steps that the change process has to go through before adoption of the change into organizational policy and culture. In this particular project, these steps will be as follows (Dearing &Cox, 2018; Pashaeypoor et al., 2016):

  1. Knowledge

This is the very first stage of the change model. It is a stage that must be handled with great care if the change project is to succeed. This is because it is the stage that introduces the staff whose buy-in is sought to the change plan. In this case, any relevant information regarding accidental patient falls and their prevention is disseminated to the employees. The motive of this is to sensitize the employees about the presence of the problem and the need for change. Sharing of this knowledge can take the form of podium presentations, posters, and departmental email amongst others.

  1. Persuasion

This is the stage in which the real work begins. Teams of innovators will be put together and dispatched to the various departments to go and actively convince their colleagues to give their buy-in for the project. For the project to succeed, they need to succeed in persuading a majority of the employees to support the change. In an organizational setup in which the interests of the organization are paramount, a carrot-and-stick approach may require to be used in this stage by obliging those resisting to either abide by organizational policy or ship out.

  • Decision

This is the stage in which the final decision to go ahead with the change is made. The decision is made after confirming that the first two steps show a likelihood of success. This decision is made by the project implementation team led by the project manager who is a nurse manager.

  1. Implementation

It is at this stage that the actual implementation of the hourly nursing rounds, installation of the bed alarms, and nurse training takes place. This stage has a specific timeframe with specific outcome objectives that must be met. In this case, the rate of patient falls has to go down substantially.


The last stage of confirmation is the stage in which the organization will now officially adopt the change into organizational policy. This is after evaluation and ascertainment that indeed the interventions are successful in reducing patient falls.
