Case Study 3:

Mrs. Simmons, an 84-year-old widow, is brought to the office by her daughter. The daughter claims that her mother seems to be depressed. There is a history of depression approximately 5 years ago, shortly after Mrs. Simmons’s husband died. At that time, she was successfully treated with antidepressants. Currently, the daughter states that her mother’s memory for appointments and events has declined severely, and she can no longer drive because she does not remember the route to the store or other familiar places. The daughter also noted that her mother’s house seemed very disorganized and dirty, there was a limited amount of food in the kitchen, and the checkbook had not been balanced for several months. THEN: Review the case study and based on the provided information, think about a possible patient evaluation plan. As part of your evaluation planning, consider where the evaluation would take place, whether any other professionals or family members should be present, appropriate assessment tools and guidelines, and any other relevant information you may wish to address.
Consider whether the assessment tool you identified was validated for use with this specific patient population and if this poses issues. Think about additional factors that might present issues when performing assessments such as language, education, prosthetics, missing limbs, etc.
Consider immunization requirements that may be needed for this patient.
Keep in mind that as nurse practitioners, whether you are primary care or acute care, patient and family education is always important. This includes education about medications, tests, treatment plans, immunizations, diagnosis, and health promotion.

Dementia is a term used to refer to a set of conditions that result from an individual’s inability to think, remember or make the daily decisions. Dementia manifest itself in many ways, with the major one being Alzheimer’s disease (Ismail et al., 2017). I agree with you that the onset of dementia symptoms encompasses behavioral changes, cognition and the general functioning of an individual. The disorder is commonly attributed to aging despite other existing factors that trigger the development of dementia. In the presented case study, I agree with you that Mrs. Simmons’ possible disorder is dementia due to the signs and symptoms that her daughter narrated. According to Simonetti et al (2020), the common symptoms of dementia include loss of memory, difficulties in executing familiar daily chores and changes in moods which are among the symptoms recorded in Mrs. Simmons case. Dementia requires immediate treatment plan when detected to prevent worsening of the condition. In such cases as Mrs. Simmons, both pharmacological, non-pharmacological behavioral and cognitive forms of treatments are required after a thorough examination. The reason is that dementia treatment plan requires individual focus of every symptom. Practitioners should however put more emphasis in cognition evaluation of the patient to help distinguish between aging and dementia considering the advanced age of Mrs. Simmons who is 84years old (Tisher & Salardini, 2019). I also agree with you that it is essential for the family members of the patient to be available during the evaluation process to help in answering some of the essential questions. Patients’ family therefore plays an essential role in the entire process of care since they are also educated on the ways of maintaining care to their patients. I agree with you that the family members of the patient should team with healthcare providers towards ensuring that the patient is not overwhelmed by these symptoms.
