Transformational Leadership in Nursing and in Health Care
Dorothy Johnson’s Behavioral System Model advocates for fostering an effective and efficient behavioral functioning that presents stresses and illness. It identifies each individual as a behavioral system that is comprised of seven purposeful, repetitive and patterned behavioral subsystems: affiliative, dependency, ingestive, eliminative, sexual, aggressive, and achievement subsystems. The functional requirements of each subsystem is protection from negative influences, a nurturing environment, and stimulation to grow. The absence of any functional requirement results in a state of equilibrium/imbalance. Through an understanding of the functional requirements, it is possible to develop a strategy for ensuring that the subsystems are in balance. Applying an effective strategy would require that an external regulatory force be applied to optimize the subsystems (Evgin & Bayat, 2020).

The application of the Behavioral System Model can help in identifying and addressing cases in which this practice issue occurs. It is unfortunate that nurses who are themselves known for having caring qualities have not been spared from this issue as they have either fallen prey or become perpetrators of these malice. In applying the theory, it is possible to perceive the issue as a social phenomenon that negatively impacts the appreciation for the uniqueness of nurses’ roles and nursing as a profession, vastness of nursing influence in the society, and the science, art and discipline of nursing embodiment and execution. Through the theory, an abductive inquiry process can be adopted to develop an understanding of the critical factors that trigger the existence of these malice and fuel their persistence (Smith, 2020).

For instance, if a nurse is frustrated and turns that frustration to anger that is manifested as curt responses to peers, then this can be perceived as nurse-to-nurse incivility. In this case, applying the Behavioral System Model would involve three steps. The first step entails assessing the situation and persons involved to determine the cause of the disruptive behavior, such as a nurse being overworked, tired and irritable. Once the cause of the disruptive behavior is determined, then the second step is to develop and apply an intervention plan, such as reducing the workload and providing social support so that the nurse is not overworked and irritable. The third step is to conduct an evaluation to determine whether the issue is resolved and the subsystems are in balance. If the nurse is helped to maintain an equilibrium of the behavioral system through the work situations, then the issue would be successfully resolved (Alligood, 2022).
With the seven subsystems acting as a frame of references, the theory allows for a personalized approach to resolving the practice issue of concern. That is because the persons concerned may have different combinations of subsystems being out of balance, such as one individual having the attachment subsystem being out of balance while another individual having the dependence subsystem being out of balance. Through the personalized approach, it is possible to identify the repetitive, purposeful and patterned ways of acting that are unique to the individual. This information is useful for presenting an external regulatory force that returns balance or at least present conditions that allow the individual to perform at optimum levels (Masters, 2022).
In addition to Dorothy Johnson’s Behavioral System Model, American Association Critical Care Nurses Synergy Model for Patient Care can also be applied. In this case, the synergy model would be applied to develop a staffing solution in which the unique qualities and competencies of nurses are matched. For that matter, this model allows for nurses to be matched in teams. This helps to create a supportive environment (Murray, 2022). Based on the model, nurses who are mismatched would be separated into different teams to reduce points of contact thereby minimizing opportunities for uncivil behavior. This model allows the nurse leaders to intervene in keeping apart nurses who may exhibit antagonistic behavior towards each other (Masters, 2022).
Transformational leadership can guide the application of the two theories. In this case, a transformation leader would act in four ways. Firstly, act as an idealized influence by acting as an exemplary role model and presenting an example of the desired behavior as a transformational leader is respected and trusted by others to make good decisions for the organization. Secondly, individualized consideration by activing as advisors and coaches who perceive each person and case on an individual basis. Thirdly, providing inspirational motivation by encouraging the nurse personnel to commit to the vision of the organization and encouraging team spirit. Finally, providing intellectual stimulation by promoting creativity, critical thinking and innovation through challenging the norms in an effort to improve the organization (Huber & Joseph, 2022).
Overall, the two theories are helpful in addressing incivility, violence and bullying in nursing practice. The behavioral system model acts on the welfare of the persons concerned by identifying the causes and guiding corrective behavioral actions. The synergy model ensures that nurse teams are synergistic and well matched while avoiding opportunities for uncivil behavior. Transformational leadership offers a framework for applying the two theories to address incivility, violence and bullying in nursing practice.
